The New Client Forms to be completed during the initial visit include:
- Legal Considerations (We know it's boring but per our insurance company it
must be signed and returned) - Veterinary Instructions & Release (Again boring, but the same as above)
- Pet Information Form (Please complete one copy for each pet; each form is 2
pages, and return) - Our Rates
- Service Request (Fill out and print one for each trip or service period, sign and
return) - Home Guide & Contact Information sheets for you to complete
Please Have These Items ready for the Initial Interview:
- Your signed documents
- A key. We will provide a key chain and a code (no name will be put on your key)
- Emergency contact information for yourself, and 2 other contacts
- A list if items you plan to leave out during pet sitter visits (such as paper towel,
medicines, etc) - Veterinary contact & medical information (allergies, conditions).
- Trip information, including Hotel and if you plan to have visitors while away.
